Consent

I consent to my child partaking in activities provided by volunteers, of ASD Ireland 
I consent to my child having their photo/video taken and used in publications. 
I consent to ASD IRELAND volunteers/staff / directors providing intimate personal care for my child in line with the child protection policy should my child require it.
I indemnify all ASD IRELANDS volunteers / board members / staff and the organisation it’s self from all liability if a child is injured while partaking in activities provided in the centre or environment
